Full Job Description

The successful candidate will be required to work independently in providing education and information, advice, and support to patients and their families/carers in their own home and within clinics, as well as remotely via phone calls or video conferencing. You will also be required to liaise and work closely with other health care professionals in ensuring that care needs are met accordingly, and that care is delivered to a high standard. You will be working as part of the integrated neurology team providing specialist nursing intervention for patients with Parkinsons disease and associated conditions. The post holder will carry out comprehensive specialist holistic assessments and use their specialist knowledge and skills to help manage the Parkinsons caseload. You will also be required to liaise and work closely with other health care professionals in ensuring that patient care needs are met accordingly, and that care is delivered to a high standard. The role with involve partnership working with the consultant neurologists and the integrated neurology team.
