Full Job Description

The postholder will work within guidelines and protocols to undertake treatment room nursing duties and assist with nurse run long term condition clinics. Have change-management skills and ability to support patients to change lifestyle. Good IT and communication skills

Practice experience is desirable.

Dale Medical Practice is a forward thinking GMS Training practice with 6500 patients. We have 3 GP Partners, 1 Salaried GP, 1 ANP and Practice Nurses, together with PCN ARRS staff we have a FT GPA and SNA and access to Pharmacist, Dietitian, Physio, Mental Health Practitioners, Social Prescriber, Health and Wellbeing Coach and support from Care Coordinators.

Due to retirement of our long standing Practice Nurse, Dale Medical Practice are looking for a Part time Band 5 equivalent Practice Nurse to support our existing clinical team. Salary commensurate with experience

The role is 20-25.
