Full Job Description

We are seeking a highly organized and proactive individual to join our team as a Pathway Coordinator for Diabetic Eye Screening. As a Pathway Coordinator, you will play a crucial role in managing various aspects of our screening clinics and ensuring the smooth operation of our service.

You will have the responsibility for monitoring dedicated pathways and their administrative procedures, which will facilitate the patient's journey and referral to Ophthalmology departments in Acute Trusts as necessary.

You will have responsibility for the quality of data and participate in regular data cleansing exercises, liaising with staff from NHFT, Acute Trusts and Primary Care.

In conjunction with the Diabetic Eye Screening team, you will support the delivery of the service,. This will require ensuring that the policies and procedures are in place and are regularly audited for compliance as directed by the Programme Manager/Failsafe Manager.

You will be working as a part of the Diabetic Eye Screening team, provide cover when required, for other team members during sickness, annual leave, breaks and other absences as required.

You will need to ensure a complete understanding of the pathways for Diabetic Eye Screening services and will assist the Diabetic Eye Screening service and Programme Manager to ensure the Diabetic Eye Screening service is constantly monitored and delivered in accordance with requirements outlined by the National Diabetic Eye Screening Programme, Slit Lamp Biomicroscopy (SLB) Clinic Pathway Management: Efficiently manage screening clinics, including booking appointments, coordinating schedules, and providing education to patients regarding the screening process and its' importance in managing diabetes-related eye complications. Keeping accurate records of attendance and reporting to the Programme Manager and Clinical Lead.

Ante-Natal Appointment Management: Coordinate and manage appointments for pregnant individuals requiring diabetic eye screening, ensuring timely and accurate reporting to the Programme Manager and the Hospital Eye Services.

Management and failsafe of newly established OCT Pathway: Oversee the management and failsafe processes of clinics and appointments in our new Optical Coherence Tomography (OCT) pathway, ensuring adherence to established protocols and standards. Educating the patients about the significance of attending these important appointments.

Covering annual leave: Provide failsafe cover during team members' annual leave to ensure continuity of services and maintain high standards of care.
