Full Job Description

Be the principal point of contact between the bowel screening programme and screening participants referred for further investigations following a positive screening test
Be responsible for having telephone conversations with participants to proactively explore and breakdown barriers to them accessing diagnostic and therapeutic procedures
Monitor the participant's positive screening pathway and work in collaboration with third-party organisations and Health Boards to ensure they have appropriate and effective support to access services and screening appointments

The Being Our Best colleague group which applies to this role is Colleague., Act as the central point of contact for bowel screening participants awaiting therapeutic and diagnostic procedures (screening colonoscopy or computer tomographic colonography (CTC))
Proactively contact screening participants to investigate any concerns or barriers to them attending for their scheduled investigative and therapeutic procedures
Respond to telephone and written queries, liaising and escalating to clinical and/or operational teams as required
Demonstrate high levels of empathy, sensitivity and professionalism in all communications with participants and their families
Assist participants in accessing appropriate information and support by signposting to a range of support services and encourage self-management where appropriate
Recognise where referral to other sources of advice and support may be beneficial to overcome any barriers to attendance (e.g., nursing, other clinical colleagues or third-party organisations)
Be aware of the potential for emotional, psychological and physical distress and symptoms that the participant may be experiencing, liaise and escalate to the specialist screening nurses or clinicians as appropriate
Liaise with clerical and clinical colleagues, as well as third-party organisations to achieve the best outcomes for screening participants

Be aware of and take mitigating actions in circumstances where there may be a barrier to communication or comprehension, including referral of any suspected capacity concerns to clinical colleagues for consideration of best interest
Planning, Improvement and Monitoring
Share ideas and good practice as well as identify opportunities for process improvement
Gather service user experience feedback in relation to the service provided
Deal with concerns/complaints as appropriate, promptly escalating where necessary to the appropriate manager
Responsible for instigating and documenting contact with clinical and clerical teams to ensure participants are managed appropriately and effectively through their bowel screening pathway
Organise own work schedule and caseload to meet the needs of the service
Digital and Information
Demonstrate advanced keyboard and IT literacy skills
Responsible for accurately documenting discussions and planned actions on the participant's digital screening record
Respect patient confidentiality and handle highly sensitive information provided by participants or clinical colleagues acting on behalf of the participant
Be aware of the limits of own practice and knowledge, knowing when to seek appropriate support or advice, and escalate any suspected safeguarding or other concerns to their line manager
Coordinate the dissemination of digital information to screening participants as required (e.g. SMS messaging)
Document and monitor all aspects of the care coordination and service delivery on relevant clinical systems
Ensure timely communication is maintained with the Health Board care providers as required, including all relevant documentation to the participant's GP
Manage the coordination and dissemination of participant resources, including leaflets and letters
Training
Undertake a period of training in the role to gain knowledge of clinical terminology, barriers to screening participation and inequalities
Gain an understanding of support available to screening participants
Provide training to inexperienced staff on the role of Pathway Navigator
Take responsibility for personal and professional development and education, through active participation in the organisation's appraisal process and development of a Personal Development Plan, including compliance with statutory and mandatory training requirements
Freedom to Act
Work to defined policies and procedures, and use own initiative, with advice and support from the line manager as required

The salary scale shown is for a full-time member of staff, with the amount being adjusted pro rata for those working less than 37.5 hours per week.
In general, anyone joining our Trust who has not previously worked in the NHS will, by default, start on the first point of the advertised pay band. Pay progression will then be determined by annual Performance and Development Reviews in accordance with the NHS Wales Pay Progression Policy. An application for Incremental Credit may be considered, but can only be awarded based on evidenced, reckonable service and/or equivalent, relevant experience.
