Full Job Description

Click on this link to find out about our values https://www.uhbw.nhs.uk/p/about-us/our-trust-values, · Work closely with the Hep C ODN team to proactively manage patients diagnosed with Hepatitis C/B, including potentially working to an unpredictable pattern of events. · Make decisions where there may be more than one solution to effectively manage and deliver the projects, including managing resources and placing orders, demonstrating awareness of maintaining budgets. · Communicate key and changing messages to stakeholders about the development of projects. · Act on results of liver scans as required & book any relevant follow-up radiology appointments. · Collect, collate, and input data into a range of local and national databases to demonstrate compliance with relevant national standard(s). · Use a range of IT applications to complete audits of project data to support ongoing project monitoring with NHS England., To provide project support to the Hepatitis C Operational Delivery Network. This will involve:

  • Working closely with Specialist Nurses, clinicians and support agencies within the Bristol and Severn Hep C Operational Delivery Network ('ODN'), to support the effective management of the programmes in order to proactively manage the project deliverables: Specifically, this post will support cirrhosis patients attending USS, setting up clinics/appointments and data collection at our Weston site.
  • Contribute to ongoing planning of projects, to ensure that the project deliverables are met within the duration of the project.
  • Help prepare documentation for MDT meetings
  • Accurately collect and record patient-level data. Maintain and enter data into local and national databases ensuring patients are seen promptly within their pathway.
  • Managing patient recall systems for radiology appointments and ensuring appropriate transportation is arranged.
  • Managing the incentivisation element of the programme., the welfare of children, young people and vulnerable adults.
