Full Job Description
This is a new and exciting post which offers an opportunity to influence the direction of this new role. This may suit someone with a formal or informal background of education delivery, supporting education delivery, coaching skills, counselling skills or experience in supporting the wellbeing needs of people.
As Patient Educator you will deliver a focused Heart Failure Education and Information Package to patients, who are newly diagnosed with heart failure and are referred into the community specialist service.
You will be responsible for visiting patients in their own home, or within a clinic setting, to provide heart failure wellbeing and education packages. Educating and empowering people with this long-term condition (LTC) to manage their health and wellbeing needs optimally. Also via the use of a Flo telehealth digital technology, which supports remote monitoring of patients with heart failure, helping them to recognise symptoms early on, while living as independently as possible.
A comprehensive induction and training programme will be provided to ensure you are fully supported, equipped and confident to fulfil your role.
This role is Mon-Fri 9am-5pm. You will be required to travel adequately and independently across a large geographical area across North Staffordshire., You will work as part of a Heart Failure multi-disciplinary team working closely and supported by the community heart failure specialist nurses and wider community healthcare teams to deliver this service to patients with heart failure.
As part of this exciting role, you will be:
· Supported to plan and deliver patient education and virtual monitoring appointments with patients.
· Deliver the approved heart failure education package, on a one-to-one basis with patients, delivering in a variety of community settings with key carers and family present.
· Deliver education utilising IT virtual platform monitoring mechanisms.
· Fully educated to deliver heart failure key symptom and monitoring training to patients.
· Support patients with lifestyle changes, advocating for other needs patients may have to the wider team.
· Gain understanding of local, national planned care delivery for Heart failure conditions in adherence with NICE guidance.
· With the clinical specialist team, you may support in wider heart failure education package delivery to key partners
· As heart failure education mechanisms are evaluated, innovative changes in service delivery are anticipated and future delivery could incorporate group Heart Failure education sessions
