Full Job Description

A fantastic opportunity has arisen for those looking for a new challenge as we have 1.93 Band 4 Flow Coordinator roles (1 Full time Permanent and 1 part time FTC for 6 months), to work alongside the ward teams to actively support efficient patient flow and delivering an exceptional patient experience.

You will need to have excellent communication skills for timely and accurate management of the patient journey, ensuring any blockages or delays are escalated for support. You will need to be confident to liaise across all professional disciplines, as you will be working in collaboration with colleagues across teams and departments to ensure investigations are progressed, EDNs are completed, TTOs made available and transport arrangements are in place, so that when a patient no longer meets the criteria to reside, they are able to move to the Discharge Lounge, if suitable, and leave the building within 1 to 2 hours.

You will also need to ensure that the hospital system and Miya board is kept up to date and that appropriate paperwork is available to the clinical staff.

You will need to be able to work in a calm, logical, organised and methodical way whilst being able to multi-task in a busy environment to manage patient pathways.

This is an excellent development opportunity in a role that will ensure that our patients receive the highest quality of healthcare in the timeliest manner., The post holder will provide support to the nurse in charge of their allocated wards in dealing with patient flow. They will work alongside the Multidisciplinary Team (MDT) to ensure that effective discharge planning and communication are maintained in order to provide a smooth, timely patient flow and discharge from hospital.

They will be responsible for pro-actively facilitating the administrative components of discharge and completion of discharge documentation within their cohort of patients.

They must be able to work on their own and as part of a MDT
