Are you looking for a new challenge? Are you passionate about high quality patient care?, To maintain high level of communication skills with all members of the multidisciplinary teams both internally and externally to Royal Papworth, maintaining service credibility and reputation.

Respond effectively and professionally to telephone/e-mail enquiries from internal and external stakeholders using tact, discretion, and sensitivity, as appropriate to the situation.

Provide and receive complex and sensitive information within all levels of the organisation with both staff and patients/relatives.

To liaise with the patient on admission to begin discharge planning. Working with the patient and family for discharge to be achieved in the morning. Completing the Admission & Discharge Assessment on patients' electronic record.

Work with the multi-disciplinary teams to promote criteria led discharge, including the use of all resources to support the process.

Provide the trust with plans of the following day's discharges and any complications.

Support the referral process for social, occupational therapy (OT), physiotherapy (PT), safeguarding, SALT, diabetes, stroke.

To act as a resource providing advice and information to clinical teams to enable patients with complex discharge requirements to meet their expected date of discharge (EDD) safely and effectively.

Manage choice of discharge discussions, supporting patients to transfer to available capacity, ensuring they understand why the transfer is vital to support ongoing reablement and rehabilitation.

We are seeking an enthusiastic Patient Flow Facilitator to support effective patient flow within an Outstanding Trust. This is an exciting time to be joining us as our team continues to expand and evolve.

If you are highly organised, can effectively communicate with staff at all levels and have patient care at the heart of everything you do then we are looking for you to take on this exciting role. Experience of working within inpatient settings is advantageous but not essential if you have the transferrable skills we are seeking.

The Patient Flow Facilitators are an important part of the team and support the ward by coordinating the patient pathway through the hospital. This involves patient communication prior to admission and effective discharge planning.

You will be responsible for first line management of ward and main reception staff so supervisory experience would be beneficial.

You should have the ability to work well under pressure in a busy clinical setting and possess excellent customer care skills.
