Full Job Description

An exciting opportunity has arisen within the Cancer Services Team for a fixed term, full-time, 37.5 hours per week, Patient Pathway Coordinator., You will be required to work closely with clinicians, specialist nurses, secretaries and other members of the Multi-Disciplinary Teams (MDT) and associated departments, to ensure that patients on a cancer pathway are proactively managed. You will be required to coordinate the patient journey from referral, through diagnostics and treatment, highlighting any potential delays in the pathway promptly., The post holder is required to work closely with the Multi-Disciplinary Team (MDT) Coordinators, Clinicians, Specialist Nurses, Secretaries and all members of the MDT team within the body site and with associated departments, to ensure that patients on a Cancer pathway are actively monitored through the patient pathway to provide a high quality, safe and compassionate healthcare service.

You will be required to track the patient journey from referral, diagnostics and treatment, highlighting any potential delays in the pathway and ensuring accurate data collection to support the process.

You will be required to highlight relevant patients to the MDT Coordinator for discussion at the Multi-Disciplinary Team Meeting (MDTM) and to ensure that the MDT Coordinator receives all relevant up to date information for the patients to be discussed in a timely manner with awareness of 'cut off' dates/times.

Candidates must have excellent organisational, communication, interpersonal skills and possess the ability to have difficult conversations in challenging environments.

Experience of working within a Healthcare setting is essential for this role alongside an in depth knowledge of National Cancer Waiting Times targets.
