Full Job Description

An exciting opportunity has arisen for an individual to provide operational management of our patient safety and risk team in delivering a highly complex and dynamic patient safety agenda for the Trust . The successful post holders will be critical to the development and implementation of the Patient Safety Incident Response Framework (PSIRF) and its associated Patient Safety Incident Response Plan (PSIRP). During the implementation of PSIRF a degree of flexibility within the role will be required as standard work under PSIRF is developed and implemented.

  • To manage our Patient Safety Response Team, ensuring high quality reports and actions are generated. Ensure we provide a patient safety response team visit for incidents. Support the delivery of the Trust's risk, quality, and safety agenda. Work closely with the Datix Team to develop patient safety dashboards ensuring analysis and presentation of patient safety/risk management information and trend data to support action and learning as part of PSIRF development and implementation. To ensure the efficient dissemination of quality and patient safety related information across the Trust. Attend meetings and present patient safety and risk information. Facilitate, in conjunction with Group Managers/Clinical Directors/Modern Matrons, specialty departmental risk assessment processes and to ensure that the risk registers are effectively populated to enable analysis and identification of trends and actions arising, and to monitor on a regular basis to ensure that risks are controlled., The Patient Safety Manager will be operationally responsible for of all aspects of patient safety and incident management functions to support the delivery of high quality, safe care for all who use the Trust's services. The post holder will coordinate and implement the NHS Patient Safety Incident Response Framework (PSIRF) in conjunction with the Head of Patient Safety and Risk. During the implementation of PSIRF a degree of flexibility within the role will be required as standard work under PSIRF is developed and implemented.
  • Creating and measuring meaningful engagement with staff and patients
  • Provide support and guidance to our Patient Safety Partners
  • To deputise for the Head of Patient Safety and Risk as and when required.
  • Provide professional advice to managers and staff within the Trust on patient safety and clinical risk management
  • Support the delivery of the Trust's risk, quality, and safety agenda
  • Support an integrated approach to all aspects of risk management, with robust systems to support this
  • Liaise with key clinical and non-clinical staff to maintain the profile of patient
  • Contribute to disseminating and supporting implementation of patient safety and clinical risk management polices related to risk and governance for the organisation, as needed
  • Contribute to the effective integration of patient safety/clinical risk with other quality governance and accountability programmes and functions
  • Work closely with the Datix Team to develop patient safety dashboards ensuring analysis and presentation of patient safety/risk management information and trend data to support action and learning as part of PSIRF development and implementation
  • Coordinate Trust responses to external bodies and the submission of evidence as required
  • Support the review, design, and delivery of training and education programmes, including teaching and presentation materials, relating to Risk and Quality and Patient Safety to meet the needs of staff working in the organisation.
  • Support the build and development of information resources pertinent to patient safety as a resource for the Trust
  • To ensure the efficient dissemination of quality and patient safety related information across the Trust
  • Be responsible for the coordination and administration of the Patient Safety Response Team
  • Work collaboratively with the Clinical Harm and Mortality Lead to ensure information is shared and considered under PSIRF
  • Attend meetings and present patient safety and risk information.
