Full Job Description

Chester Central PCN is a collaboration of 6 GP Practices in Chester City centre who collaboratively care for over 40,000 patients. We are looking for a PCN Administrator to join our leadership team. The post-holder will work closely with the PCN Manager and Digital Transformation Lead and Co-Clinical Directors., Understanding all relevant PCN contracts both local and national.

Supporting the development of excellent relationships across the network to enable collaboration for better patient outcomes.

Supporting the PCN Manager & Clinical Directors (CDs) to provide strategic leadership for workforce development, through assessment of clinical skill-mix and development of network workforce strategy.

Supporting network implementation of agreed service changes and pathways, working closely with member practices, the wider PCN and the commissioner to develop, support and deliver local improvement programmes aligned to national and local priorities.

Developing relationships and working closely with other local teams, health and social care providers, local commissioners, and Local Medical Committees (LMCs).

The post holder will have excellent communication skills, keep confidentiality at all times and be flexible in their approach. The right candidate will be confident gathering and analysing data and in learning to use new computer systems.

The Administrator role is an exciting opportunity to join us and be part of the delivery of the GP DES Contract helping us to shape and inform innovative and exiting new services in Chester Central PCN. You will have a very important role helping support the PCN agenda and delivery of care.

In this role you will be answerable to the PCN Manager, and your main task will be in the day-to-day support of this role and of the Clinical Directors.
