PCN Administrator

The role may provide additional support to the ongoing efforts of Spring and Winter Covid19 vaccination programs as well as other seasonal PCN activities. The role will work alongside other PCN clinicians and AHP's to support the administration of the local care homes assigned to the PCN as well as being responsible for new patient registrations. Principal Responsibilities 1. Provide support and encourage engagement and attendance on LD and SMI Reviews. 2. Provide additional capacity for Practices by supporting the uptake of NHS Health checks in addition to other clinical duties as directed by the Practice teams. 3. Ensure patients who are referred to the Social Prescribing service are appropriately triaged within 3 working days and directed as per the agreed pathway.
