Full Job Description

We are currently looking to recruit a PCN Care Coordinator on a permanent contract to work full-time. The role will entail work across Grafton Medical Partners and Lambton Road Medical Practice, but will primarily be based at our Trevelyan House site in Tooting.

The Care Coordinator works in support of both clinical and non-clinical team. The ideal candidate is required to assist with the management and development of patient care coordinating duties listed below as well as liaising with internal and external stakeholders including but not limited to; district nurses, palliative care, social services and any other third party agencies.,

  • · Process and effectively signpost patients to the appropriate healthcare professional depending on the presenting condition.

  • · Answer incoming phone calls, transferring calls or dealing with the callers' requests appropriately.

  • · Initiate contact with and respond to requests from patients, team members and external agencies.

  • · Photocopy documentation as required.

  • · Scan patient-related documentation and attach scanned documents to patients' healthcare records.

  • · Action incoming emails - direct to relevant team, scan onto patient's healthcare records.

  • · Input data into the patients' healthcare records as necessary.

  • · Processing all Two Week Rule (TWR) referrals, monitor and run regular EMIS reports accordingly.

  • · FIT tests - maintain a clear record of FIT tests requested, chase ups, liaise with GPS where necessary.

  • · Carry out system searches as requested.

  • · Responsible for Acurrx and Website Dashboard - liaise with clinical and non-clinical staff to resolve patients' requests.

  • · ERs support - manage all sections accordingly as well as organise and maintain framework for monitoring on a regular basis.

  • · Children's Safeguarding:


  • - Support Child Safeguarding Lead - attend meetings, chase up cases through Social Services.

    - Run EMIS searches for babies' 8, 12 and 16 weeks checks.

    - Code Bloodspot results on EMIS.

    - Assist on-call Dr with incoming requests from Social Services for completion of Section 17 and Section 47.
  • · Work with Registration Manager re data entry of new and temporary registrations and relevant patient information as required.

  • · Assist Senior Administrator with directing requests for information, e.g., SAR, insurance/solicitors' letters and DVLA forms, to the relevant team.

  • · Support Medical Secretary with referrals as required.

  • · Support Prescription Administrator when needed in the management of repeat prescriptions, ensuring that they are processed accurately and efficiently.

  • · Support all clinical staff with general tasks as requested.

  • · Manage all queries and responsibilities in an efficient manner.

  • · Undertake mandatory training and induction programmes..

  • ·Attend weekly meetings with Admin Lead.
