PCN Care Coordinator

OurDudleyWidePCNsarelookingforaCareCoordinatorwhowillwork acrossthewholePCN, supporting our patient population and their personalised care requirements, supporting the PCN with triage and successful care navigation.

Thecandidatewillneedaflexibleapproachinthedeliveryof thePCN'saimsforpatient centred care. The role will require movement around all of Practices so access to a car is essential.

Main duties of the job

1)Identifyaperson'scareandsupportneeds andexploretheiroptionstomeettheseintoasingle personalised care in collaboration with the patient's primary care team.

2)Provide coordination and navigation for elderly people and their carers across health and care services,alongsideworkingcloselywithsocialprescribinglinkworkers,occupationaltherapists,dietitians.
