PCN Clinical Pharmacist / Care Homes Pharmacist

The post holder will work within their clinical competencies as part of a multi-disciplinary team to provide expertise in clinical medicines optimisation across the PCN. This will include structured medication reviews with direct patient contact and may include contributing to: Management of long-term conditions To support the care homes within the PCN with structured medication reviews To work with colleagues to establish and develop good medicines management systems across the care homes sector To assist the PCN care homes team and wider MDT with care home-related medication queries Liaise with care home staff as necessary to ensure accurate exchange of information and clarification of medication queries Work effectively within the PCN Pharmacy and Care Home Teams across all practices within the PCN To establish and improve professional standards of care in the management of residents within the care homes by developing relationships with key stakeholders Contributing to repeat
