Full Job Description

We have an exciting opportunity for an Enhanced Clinical Practitioner to join our team, where you can make a positive contribution to patient care and peer support and supervision. As an Enhanced Practitioner you will be acting within your professional boundaries, providing care for patients from initial history taking, clinical assessment, diagnosis, treatment, and evaluation of their care with the support of the PCN Clinical Lead as required. You will demonstrate safe, clinical decision-making and expert care for patients within the Primary Care Network (PCN), whilst working autonomously and taking full accountability for patient encounters. The Enhanced Clinical Practitioner will undertake duties within the Practice itself thus ensuring contemporary clinical skills and credibility as well as ongoing integration with the wider team and services. This would include review and care of patients attending the Practice (for pre-planned services or same day urgent care) and include diagnosing and treating patients presenting with minor illness, seeing, and advising people in respect of their continuing medical and nursing needs. Your work will demonstrate improvement in patient outcomes, contributing to reduce health inequalities across the PCN and improving the efficiency of general practice and delivering a best-in-class service.,

  • 1 An ability to make professional autonomous decisions for which they are accountable.
  • Assess and triage patients, including same day triage, and as appropriate provide definitive treatment (including prescribing medications following policy, patient group directives, NICE (national) and local clinical guidelines and local care pathways) or make necessary referrals to other members of the primary care team.
  • Screen patients for disease risk factors and early signs of illness. Use complex decision making to inform the diagnosis, investigation, complete management of episodes of care within a broad scope of practice.
