Full Job Description
We are seeking a dedicated and compassionate Enhanced Practice Nurse to joinour PCN healthcare team. The Enhanced Practice Nurse will play a vital role inproviding high-quality patient care, assisting in the management of chronicdiseases, and supporting the overall health needs of our patients. Thisposition requires a strong understanding of nursing practices and a commitmentto delivering exceptional care in a clinical setting. Main duties of the job Develop effective working relationship withPCN member practices. Facilitate and support the development anddelivery of innovative training and education to staff, to improve competenceand confidence, in line with best practice and guidance related to area ofexpertise. Participate in and or lead formal trainingevents promoting best practice in area of expertise. Provide professional and clinical leadershipand mentorship, acting as a clinical role model on the delivery ofevidence-based practice. Advise and support on the development anddelivery of work streams relating to area of expertise as part of the PCNsstrategy, where appropriate. Work proactively with key local and nationalstakeholders to develop more integrated care pathways and holistic models ofcare, through dissemination of specialist skills and knowledge. Contribute to the delivery of local policy,procedures and protocols for the PCN to improve identification, assessment,ongoing support for patients and their family carers. Consults with patients, their family, and themulti-professional team to undertake assessments of patient need and devise andevaluate complex care plans., Delivers enhanced clinical care in the contextof continual change, challenging environments, different models of caredelivery, innovation and rapidly evolving technologies using analysis and theirunderpinning knowledge to manage complex interventions.
- Registered Health Care Professional (eg HCPC - NMC)
- Has a post graduate qualification at level 7 or above relevant to their area of enhanced practice, for example in wound care, diabetes plus respiratory and CVD, dementia, COPD, Asthma.
- Has worked within primary care in services that enhance the core general practice offer.
- Working at an enhanced level of practice as described in the Primary Care and General Practice Nursing Career and Core Capabilities Framework.
- Knowledge of audit - research and experience of analysing findings to improve practice.
- Knowledge and understanding of clinical governance.
- Demonstrate the ability to work in a highly demanding environment.
- Ability to work independently and make autonomous clinical decision within own area of competence.
- Evidence of continuing professional development.
- High level communication skills, both written and verbal.
- Ability to reflect on own practice. Experience Essential

- In depth knowledge of anatomy to effectively understand patient needs and conditions.

Our PCN is made up of 4 practice The Mounts Medical Centre, Favell Plus, Maple Access and Mayfield Practice, you will berequired to work in all four clinical sites during the week.
