Full Job Description

The Medics PCN have an exciting opportunity for a permanent nursing associate to join our team on a full-time basis at our Marsh Farm Health Centre. You will be a valued member of our large and established multi-disciplinary team made up of clinical pharmacists, first contact physiotherapists, paramedics, nursing associates and a vast personalised care team. Duties will include;

  • Working within the PCN to provide and monitor care, alongside the wider PCN team.
  • Contributing to the delivery of excellent patient care, supporting the existing workforce under direct or indirect supervision.
  • Supporting registered nurses and the delivery of nursing care for the practices and the PCN.
  • Performing and recording clinical observations such as blood pressure, temperature, respirations, and pulse.
  • Undertaking additional training, provide flu vaccinations, ECGs, and venepuncture, and other relevant clinical tasks as required by the PCN.
  • Maintaining accurate and contemporaneous patient health records.
  • Supporting with chronic disease clinics and maintaining registers, deliver patient care, referring patients to other members of the PCN or to secondary/specialist care as appropriate.
  • Implementing and evaluating individual treatment plans dependant on training and experience.

    Completed the two-year foundation degree, recognised by the Nursing and Midwifery Council.
  • Registered with the NMC and revalidation is undertaken in line with NMC requirements.
  • Demonstrable commitment to professional and personal development.
  • Knowledge of the needs of vulnerable adults, safeguarding and the associated legislative framework.
  • Understanding of basic health and social care terminology.
  • Good interpersonal skills.
  • Ability to communicate confidently with staff of all seniority levels.
  • Ability to work well across teams.
  • Good time management and prioritisations skills, ability to work to strict deadlines.
  • Ability to work on own initiative without direct supervision, understanding where clinical input is needed, recognising own limitations.
  • Desirable:
  • Familiarity with IT systems currently in use in the Medics PCN and its member practices, SystmOne.
  • Experience of health and social care assessments.
  • Demonstrated successful outcomes delivery within determined timeframes.

The Medics PCN has 5 active and engaged member practices which are: Woodland Avenue, Medici Medical Centre, Bell House Medical Centre, Gardenia & Marsh Farm Practice and Barton Hills Medical Group.
