Full Job Description

The aim is to provide a service across ARC MK integrated drug treatment service with a focus on the homeless, rough sleepers and hostel residents that reside in the Milton Keynes.

The post holder will use their specialist foundation knowledge and skills to undertake assessment, care planning, implementing and evaluating the case and treatment of clients.

He/she will perform duty assessments to clients that are new to the service, in addition to acting as keyworker to a defined allocated caseload of clients with a low to medium degree of complexity, under the supervision of designated Senior Nurse within the team.

The post holder will be responsible for ensuring that the delivery of the care and management of this client group is done in compliance with HCC Quality Standards., An exciting opportunity has arisen to join the rough sleepers and homeless outreach team as part of the integrated drug and alcohol treatment service in Milton Keynes

We are seeking practitioners or nurses either with experience working with those with substance misuse issues or someone looking to expand their practice into a new field and embrace new a new challenge. You will join our multidisciplinary team to help engage with the homeless population who use drug and alcohol to help engage in community-based treatment to secure long-term accommodation.

The role will be part of the Addictions Recover Community, Milton Keynes (MK) team, but predominantly community-facing, working in sites across Milton Keynes.

CNWL Addictions is a well-established NHS provider offering a wide range of high-quality addictions treatment interventions to the diverse and multicultural population across Brent, Ealing, Hillingdon, Hounslow, Milton Keynes and HMP Woodhill. In addition, we have the Club Drug Clinic coving Kensington and Chelsea/Westminster as well as our National Problem Gambling Service and Gaming Clinic. CNWL also have a nurse-led homeless peripatetic nurse team as well as specialist nurses working across a number of General Hospital sites

The service is committed to a harm minimisation philosophy and to partnership working. Our main partners are primary care services, social and the third sectors.

Treatments are provided across all our clinical centres. A Care Quality Management framework is well established and the service prides itself in its continuous development and as a learning organisation.
