  • In Adult Social Care, we take a strengths-based approach and work towards maximising independence. The Preparation for Adulthood & Transitions Team and Direct Payments Teams are citywide services in Manchester which works closely with key stakeholders in children's and education services, health and the voluntary, community and social enterprise sector to support and plan the preparation for adulthood journey with young people who are likely to require care and support from Adults Social Care, when they reach adulthood.

    This role is an exciting opportunity to work across these services to play an integral role in supporting our warm welcome approach to adult social care services, providing accurate and timely information, advice, and guidance to people in Manchester. There will be scope for the role to focus on different project areas, including but not limited to:
  • Supporting how we work with care leavers

  • Actively promoting the use of direct payments in innovative ways

  • Using the local offer to support young people with additional needs

  • Developing our approach to transitional safeguarding


  • The successful candidate will work across either the Preparation for Adulthood & Transitions Team or Direct Payments Team to support and coordinate the delivery of a range of projects of varying complexity to achieve better outcomes for people and their families in Manchester., As a Disability Confident Employer, we provide support for employees with physical or mental-health conditions, a disability or neurodiversity.     You will need to be well organised with proven experience of planning and managing competing priorities. You will have some experience of working with young people and/or adults with additional needs You will have good communication skills as a key element of the role is the ability to build relationships and work with Manchester citizens and partners across health and social care to interpret and deliver defined plans and activities. Opportunities will be provided within the programme for the successful candidate to develop within the role.
