Full Job Description
- To support and enable young people who are currently in care, and those who have left care and are eligible for leaving care services, to make a successful transition to adulthood through practical and planned interventions.
- To provide a comprehensive and effective service, within Southwark's practice framework, assessing need, and planning and working purposefully with young people. To support our young people, their networks and professionals, to manage risk, safeguard young people, and promote positive outcomes.
- To provide a service which is relationship-based and works collaboratively with our young people, centred around our young people, and which responds in a timely manner within statutory guidelines, working collaboratively with the team., Work directly and in partnership with the young person to provide a clear and accurate assessment of the young person and their needs, in line with statutory guidance. Undertake interviews, observations, and gather information from young people, carers, family and friends, and other professionals as directed by the Team Manager.
- Based on assessed strengths, needs and risks, devise, implement and review pathway plans, collaboratively with the young person.
- Manage the caseload as directed, and organise and plan work activities, prioritising tasks and responsibilities.
- Provide written and verbal reports, which are concise, informative and based on analysis of complex evidence, for a range of audiences, including young people, reviews, panels and other formal meetings, and court.
- Work with key professionals from the department and externally to offer advice and implement pathway plans. Contribute to professional discussion and challenge in an appropriate manner.
- Identify the impact on young people of their own or other's mental ill health, substance misuse, domestic abuse, physical ill health and disability, including those that may indicate risk. Access manager support and the help of other professionals to ensure young people's best interests are prioritised and they are safeguarded.
- Attend and contribute to young person statutory review and any child protection / adult safeguarding meetings, including Court as appropriate.
- Undertake the statutory role of personal advisor as defined in Care Leavers Act 2018, Care Leavers' Regulations 2010, The Children Act 1989 Guidance and Regulations, Volume 3 : Planning Transition to Adulthood for Care Leavers, and the Children and Families Act 2014.
- Develop and maintain close working relationships with key agencies to ensure a coordinated response to young people.
- Work to establish supportive and effective rapport with young people, both individually and within groups of young people as required. Maintain a professional, respectful and honest positive relationship with young and with their networks, to develop trust and assist in promoting positive and timely outcomes11. Ensure pathway plans are up to date and reviewed in line with statutory requirements, that all statutory duties are upheld and young people's rights respected and promoted.
- Maintain working knowledge of issues that impact on young people in careleaving care including benefits, housing and training and employment opportunities, statutory guidance, best evidence, and research knowledge.
- Produce written reports, maintain records and data as required, ensuring that case files are organised and up to date.
- Be involved in department, service and team development and training opportunities, contributing to organisational development, and helping to sustain a learning culture.
- Ensure all work undertaken considers the diverse needs of care leavers, and their wishes and feelings.
- Work with a degree of independence without the need for close supervision, whilst recognising when and how to seek advice from managers and senior colleagues. Prepare for and use supervision, and actively engage in performance appraisal systems.
- Contribute experience and expertise to group supervision processes, to support evidence-informed practice.
Knowledge of the health, education, training and work experience needs of young people and how these can be met. - An understanding of the needs of children looked after, and care leavers, including adolescent development, the impact of trauma, transition to independence, environmental factors, and risk and protective factors.
- Knowledge of relevant legal and regulatory framework relating to children, young people and families, particularly leaving care regulations.
- Knowledge and understanding of the problems and issues faced by people living in an inner city, multiracial / cultural community.
- Knowledge of roles and responsibilities of key children's agencies.
- Understanding of systemic practices, and restorative and attachment- informed approaches.
- Experience of working directly with vulnerable children or young people, in a professional or voluntary capacity, implementing effective interventions with successful outcomes.
- Experience of group work with young people and delivery of intervention programmes, with successful outcomes.
- Computer literacy skills at a level to maintain case records within information management systems, and ability to use technology to support online communication.
- Experience of working effectively as part of a team and balancing personal workload.
- Good verbal and written communication skills.
- Ability to develop and maintain professional relationships with young people their careers and families.
- Ability to recognise and respond appropriately to issues of risk and safeguarding.
- Ability to present complex information clearly and concisely, producing high quality reports with clear recommendations, that are easily understood by others.
- Ability to organise own workload and manage time.
