Full Job Description

Do you want to take part in meaningful work which provides emotional fulfilment, personal growth, and community engagement?
An individual requires support with enhancing their life.

Preferred days/times:
5 hours per week, Monday - Friday 11am-12 noon
2 hours to be used flexibly to access the community between Monday-Friday
Can be directly employed at £12.44 per hour or self-employed at £16 per hour

Tasks/duties:
The client requires support with preparing and cooking meals day to day, as he can experience significant pain and flare ups of FND.
The client has a walk-in shower with bariatric shower chair which he uses in the shower or to sit at the wash basin. He requires assistance of one with washing due to muscle contractures and requires assistance to put on socks and shoes. He requires minimal support with dressing.
The client is not currently going out unless it is to a hospital or doctor's appointment, and he wants to go out into the community again.
The client is unable to get his electric wheelchair in and out of his mobility car himself and requires support to do this.

Client main diagnosis:
Crohn's Disease - the client experiences flare ups and has had nine in the last year.
Functional Neurological Disorder diagnosed around 3 years ago.
Dystonia
Non-epileptic seizures - the client is aware when these will happen and will lie down on the sofa. No intervention from the PA required.
Bipolar- The client can experience extreme mood swings
Dissociative psychosis- The client has displayed disconnections from himself such as not feeling pain
Nerve degenerative disorder - Nerves in his body feel like they are burning.
Borderline Personality Disorder - The client describes this as ADHD
Hemiplegic Migraines
History of falls - Hasn't had a fall for several months.
The client currently has nine sets of professions involved in his care and treatment including Neuro physio, CPN and a psychiatrist; he can find the phone calls and visits overwhelming and so may not answer the phone or door. He will listen to a voicemail and respond to this.
He requires support with accessing the community due to feelings of embarrassment and anxiety when out in public by himself.

Client enjoys:
Going out into the community, going shopping, visiting local cafes, and being able to walk his dogs. The client used to write a blog and would sit for hours in a café, and this is something he would like to do again.
He is also very creative and enjoys crocheting, needle felting and cooking.
There are two little dogs at the property that sometimes bark at visitors but are very friendly.

Communication:
The client can communicate clearly and is able to let their wishes known
The client likes to make his own decisions and likes people to be straight to the point.
He would like the PA to respect his house and not enter his bedroom and would like to let things progress naturally, and for him to decide how much he wants to do.

Moving and handling:
The client is unable to get his electric wheelchair in and out of his mobility car himself and requires support to do so.
The client has a bariatric perching stool in his kitchen, and he will also use his electric powered wheelchair in the property.
