Full Job Description

The role of the personalised care pathway tracker will provide support to the colorectal cancer nurse team by proactively tracking patients on a personalised stratified follow up pathway, and supporting any additional tracking needs for surveillance pathways where required. Work with the colorectal clinical nurse specialists to support patients undergoing a colorectal cancer diagnostic pathway, patients with a cancer diagnosis or patients living with and beyond cancer. The post holder will provide tracking for patients through the delivery of personalised care following a cancer diagnosis., The main duties of the role will be to provide tracking support for patients on the personalised stratified follow up pathway or other surveillance pathways within the colorectal service provision. To ensure that appropriate requests for investigations are managed in a timely fashion. If diagnostic delays occur or appear imminent it is the responsibility of the post holder to escalate to the relevant manager. To be fully conversant with patient pathways across site specific tumour groups to ensure that patients are treated equitably, reaching targets for holistic needs assessments and diagnostic tests. To provide support to the team to ensure continuous development, implementation and monitoring of changes to the patient pathway.
To be conversant with all reports available on the Somerset database to manage the patient pathway. Ensuring patients on a personalised stratified follow up pathway are actively tracked, inputting of data collection and next steps of the pathway actioned. To maintain up to date knowledge of all targets and processes relating to the NHS Long term plan for cancer and to be a point of reference within the department for the Cancer Services team. Support with the development of survivorship and wellbeing services. The role will include working in a team environment and to provide appropriate support/advice to other secretarial/clerical staff as necessary as well as supporting the delivery of Macmillan information to patients and carers., The role of the personalised care pathway tracker will provide support to the colorectal cancer nurse team by proactively tracking patients on a personalised stratified follow up pathway, and supporting any additional tracking needs for surveillance pathways where required. Work with the colorectal clinical nurse specialists to support patients undergoing a colorectal cancer diagnostic pathway, patients with a cancer diagnosis or patients living with and beyond cancer. Work closely with clinicians, specialist nurses, admin staff and all members of the Multi-Disciplinary Team to ensure that patients are actively managed through the cancer pathway. The personalised care pathway tracker will provide tracking support of patients through the delivery of personalised care following a cancer diagnosis. Support and escalate recurrence of cancer to the nursing team. Support ongoing wellbeing programmes.
To work closely with clinicians, specialist nurses, secretaries and all members of the Multi-Disciplinary Team (MDT) across all specialties to ensure that diagnostic tests have been requested and work with the Cancer Nurse specialists to manage coordination of results and appointments. To ensure that appropriate requests for investigations are managed in a timely fashion. If diagnostic delays occur or appear imminent it is the responsibility of the post holder to escalate to the relevant manager. To provide support to the team to ensure continuous development, implementation and monitoring of changes to the patient pathway. Escalating any diagnostic test results that require action to the cancer support worker. To maintain up to date knowledge of all targets and processes relating to the NHS Long term plan for cancer and to be a point of reference within the department for the Cancer Services team. Provide support and development of survivorship and
wellbeing services. To identify delays in the patient pathway and to follow the trust escalation policy to raise awareness to the Team Lead for Macmillan and Lead Cancer Clinician as appropriate. Build relationships with a wide range of staff both internally and externally to undertake job role. Liaise with all diagnostic departments internally and externally to ensure that patient's tests are booked, undertaken and reported. Ensure all interactions with patients and staff are accurately recorded on the Trust's Cancer Register (Somerset).
Ability to promote patient empowerment and self-management strategies. Deliver self-management advice, support and know when to escalate to clinical team.
