Full Job Description

Manage and promote the repeat dispensing serviceRepeat prescription reauthorisation and acute prescription requestsMedication related queries from patients and staffIncrease safety mechanisms such as clinical audits

Implement MHRA drug withdrawals and alerts

Reduce inappropriate poly-pharmacy and wasteful prescribing through clinical medication review

Contribute to reductions in medicate related hospital admissions and readmission by supporting patients to get the best outcome from their medicines and identifying and addressing medicine related issues

Reconcile medicines following hospital discharge and work with patients and communitypharmacists to ensure patients receive the medicines

Work within the Group to undertake medication reviews particularly in high risk groups such as frail elderly, substance misuse and patients on high risk medicines

Improve prescribing practice through educational support for all prescribers within the practice

Run and oversee the YMG Anticoagulation service using the surgerys protocol

Lead on where changes in evidence require changes in prescribing across patient population eg where a drug is withdrawn or indications change

Ensure patient safety when they are transferred between care providers through reconciliation of prescribed medicines

To attend and contribute to meetings as requested, The pharmacy team drive the focus on reducing some of the burdens placed on the GPs by promoting the electronic repeat dispensing service, reducing inappropriate polypharmacy and wasteful prescribing through medication reviews, working with the complex care team to review care home patients, increasing safety mechanisms through audits, implementing drug withdrawals and alerts, and contributing to reductions in medication related hospital admissions and re-admissions by supporting patients to get the best outcome from their medicines.

In addition to the day-to-day duties, the successful candidate will be trained to oversee the anticoagulation service and will also be encouraged to develop in another clinical area of interest. They will also be expected to be proactive, flexible, possess good time management skills and be able to prioritise workload.
