Join one of London's leading Trusts and be part of a progressive and dynamic environment where you can build a rewarding career in pharmacy as a Pharmacist working within our Acute Medical Units (AMU), Acute Assessment Units (AAU) and Elderly Medicine wards at Charing Cross and St Mary's hospitals. You will be part of one the largest teams in our Pharmacy Department, already comprising of a group of extremely friendly and aspirational people. You will play a pivotal part in our services, providing support and working closely with medical, nursing, pharmacy staff and the wider multidisciplinary team (MDT) on a regular basis. Alongside attending ward rounds, you will also have the opportunity to partake in various MDT teaching, audits and service improvement projects. Each site will provide unique insights and opportunities for your own development and to collaborate with our fabulous teams.,

  • To integrate research evidence into practice
  • To plan, monitor and review drug treatment protocols including implementation of new drugs and treatment strategies
  • To optimise patients knowledge of their drugs including counselling patients and relatives on the use of their drugs
  • To enhance the quality of patient care
  • To ensure that patients admitted to Emergency and Elderly medicine wards have accurate drug histories documented, any supplies of their own medication checked, and any medication required supplied as soon as possible.
  • To ensure that all changes to medication are communicated to the GP
  • To support the lead pharmacist in leading on falls prevention
  • To demonstrate expert clinical knowledge
  • To influence prescribers and recommend changes to patient's treatment where appropriate
  • To work with a whole-system patient focused approach
  • To communicate with patients and health care professionals
  • To prioritise clinical interventions according to individual patient needs
