Full Job Description

This is quite a new role in a relatively new PCN organisation, so were looking for someone with the flexibility to shape with us the support we need hence the wide scope and flexible nature of this role. This is so much more than a traditional Pharmacist role as it covers critical areas of Pharmacy work in our GP Practices. The Pharmacy First support role is to be carried out over a minimum of 3 clinical sessions per week. This document outlines the overall objectives for the Pharmacy Lead role that has been introduced for 2024/25 to form part of the overall Change Management Programme supporting General Practice. This document gives an overview of the roles and responsibilities of the pharmacy lead to support the development and uptake of Pharmacy First, in line with the Primary Care Access Recovery Plan (PCARP)., The Place-based pharmacy Lead will be required to:

  • Be the Place named lead for implementation for their designated Place(s)
  • Ensure a minimum of 1 support session/workshop is offered at Place/PCN level per quarter between 04/2024 03/2025 for both general practice staff and community pharmacy staff
  • Ensure providers that are not engaged in the service or who have low referral/acceptance rate are visited and support activity is delivered
  • Submit timely KPIs (as per below) to programme leads
  • Analyse and/or audit Pharmacy First data and user insights to strategize future solutions to maintain/increase the service referral rate
  • Ensure community pharmacies and general practice teams are supported for any Pharmacy First related enquires provide training sessions where appropriate
  • Stakeholder engagement which may include hosting webinars, attendance of meetings at a local level either face-to-face or online (as required)
  • Attend regular meetings with SWL ICB Pharmacy First Lead and report back to the group during SWL-led meetings that will take place fortnightly.
  • Undertake any other duties appropriate to the role

    Sutton has a population of approximately 200000 residents registered to 21 practices and there are currently 4 Primary Care Networks each serving a population of approximately 50000 patients. Our Sutton PCNs are forward-looking, friendly, and focused on providing a wide range of excellent healthcare services to patients in Sutton and the surrounding area. Our PCNs between them are led by 8 PCN Clinical Directors.
