Full Job Description

  • To have an understanding of the homecare pathway, from deciding appropriateness of homecare registering patients, through to final processing of invoices.
  • Work in accordance of the homecare pathway to support the clinical teams with their homecare service
  • To process authorise and receipt Homecare deliveries on the pharmacy system
  • Work in accordance with and develop services in line with the Royal Pharmaceutical Society Standards for Homecare.
  • To be a point of contact within the Trust regarding medicines home delivery service for Homecare companies, Pharmacists, Clinicians, Purchasers, Contracting and Finance.
  • Co-ordinate the pharmacy homecare medicines service, prioritise and manage the workload and resolve problems as they arise.
  • Liaise with homecare service providers to review and prepare service level agreements for implementation within the Trust.
  • Support service review meetings for homecare medicines management working closely with homecare companies, pharmacists, nurses, doctors, other healthcare staff and patients.
  • To assist the Pharmacy Homecare Manager in the governance of Homecare through policy and protocol development
  • Assist the Homecare Pharmacy Service Manager in maintaining and reporting of departmental Key Performance Indicators (KPIs)
  • Ensure all patient related information is secure and complies with current guidance for information governance.
