Pharmacy Technician

To support the pharmacist with medicines reconciliation in accordance with practice protocols and to update patients medical records / prescribing accordingly ; -Supporting the practice based pharmacist to provide medication reviews and Structure Medication reviews; -Supporting, as determined by the PCN, medication reviews and medicines reconciliation for new care home patients and synchronising medicines for patient transfers between care settings and linking with local community pharmacists. -Assisting in the delivery of medicines optimisation and management incentive schemes and patient safety audits in line with CQC search criteria. - Support the PCN and practiceto deliver on DES specifications, QOF and other locally commissioned enhanced services -Duties in line with scope of practice to support demand and rapid change of general practice.
