Full Job Description

You will become part of the MDT in delivering a quality service to our older adults and will include:-

Undertaking medicines reconciliation for new patient admissions.

Being responsible for routine medication rounds (principally breakfast and lunchtime) in accordance with Trust Medicines Policy.

Engaging with patients , carers and colleagues in discussions about medication, providing information and answering simple medicines management queries. This could be either via planned sessions or ad-hoc interactions.

Escalating queries, as appropriate, to ward pharmacist, medical or nursing staff.

Ensuring patient requirements for stock and non-stock drugs are ordered from the pharmacy hub before existing ward supplies are exhausted.

Working with ward medical and nursing staff to ensure discharge (or leave) medication requests are ordered and processed in a timely fashion.

We are excited to add a Pharmacy Technician to the ward team and hope you will take both an active role in medication administration but also in increasing understanding of pharmacology use in this age group. You will have the opportunity to deliver education groups, individual pharmacology sessions with our patients, carer support, and work alongside our staff to improve systems, processes and quality. We offer flexible working patterns and will consider part time hours.
