To support the delivery of safe, effective, and efficient systems for repeat prescribing, medicines optimisation, reducing medicines waste and maximising patient outcomes .In addition, the post holder will be responsible for encouraging the development of better understanding of the principles of medicines optimisation throughout the practice teams and promoting good practice in line with therapeutic developments.

Castle Gate Medical Practice (formerly Chippenham Surgery) has been providing medical care to local patients since approximately 1840. Since then we have grown to provide care for a list size currently standing at approximately 14,260 patients - and increasing. We have a branch surgery in Raglan (9 miles from Monmouth). We have 9 GPs working at our Practice in addition to a great team of other clinicians, receptionists, admin and support staff. We encourage our healthcare professionals to develop their own professional development plans, allowing individuals time to teach and study. We are a registered training Practice for Registrars and medical students.
