Pharmacy Technician (Ward Based Medicines Management)

The Medicine Management Technicians will provide a ward based Medicine Management and Optimisation Service, including medicine reconciliation, patient counselling and ensuring the quality, safety and timely supply of discharge prescriptions.,

• Ensuring the quality, accuracy and safety of prescribing in exciting clinical areas.
• Support the MDT, including pharmacist, doctors and nursing staff.
• Obtaining detailed and accurate medication histories from multiple sources and reconciling with the inpatient prescription and communicating any discrepancies.
• Communicating detailed information to patients & carers about drug doses, indications, side effects and efficacy.
• Liaising widely (e.g. CCG or GP staff, patients, carers etc) to ensure the timely availability of patient medication.
• Reviewing inpatient and discharge prescriptions to ensure appropriate prescribing and availability of medicines.
• Assessing Patients own Drugs for suitability for use in the Trust.
• Promoting use of Trust and national clinical guidelines and optimising patient medication.
• Help support quality, safety and timeliness of discharge and to facilitate effective communication of medication around admissions and discharges.
• Provides a safe and efficient supply of pharmaceutical products to patients. This may involve dispensing and final checking (ACT) of drug products and compliance aids.
