We have some great opportunities for aspiring Health Visitors to move into team leader roles. Due to further investment in the leadership of the Health Visiting Team in West Hertfordshire ( Dacorum and St Albans) we have a full time Team Leader post available for you.
You will be leading and supporting a skill mixed team to effectively and safely deliver the Healthy Child Programme. You will be proactive and innovative, like a challenge and enjoy working as part of a team. You will be supported by senior leadership and the wider Trust to grow and thrive in your Team Leader role. This role is non-clinical but your work will continue to benefit child and families and you will retain you SCPHN qualification, and have access the iHV and SAPHNA for inspiration amongst other benefits., You will contribute to the strategic direction, supporting the continued integration of pathways, and take a lead role in facilitating effective positive partnerships with key stake holders.
You will lead on productivity, efficiency and clinical outcomes across the Public Health Nursing 0-5 team and will implement improvement plans where appropriate.
This role is 100% management and leadership with no allocated clinical time., + Be responsible for the day-to-day management and leadership of the team ensuring adequate staffing levels are maintained and a high standard of care provision is achieved to meet client expectations.
+ Ensure the policies and procedures of HCT are implemented and adhered to at all times and that team members are familiar with them.
+ Responsible for ensuring that staff perform effectively and safely, meeting the requirements of the post, taking appropriate action as necessary, and reporting to the Operational Manager.
+ Participate in the recruitment, selection and interview process. Contribute actively to the promotion of Public Health Nursing as a profession.
+ Lead on the continuous professional development of the respective workforce, supporting the Operational Manager with the development of services, performance capability, governance initiatives and the development of policy and practice.
Operational Delivery
+ Identify the health needs of the team's 0-5 year's population and establish priorities in line with the service specifications for Public Health Nursing, targeting vulnerable groups, in line with local and national initiatives. Actively engage with Family Centres to deliver agreed levels of service.
+ Initiate, maintain, organise and manage programmes of health surveillance to include health reviews for "Children Looked After".
+ On a daily basis have due regard for personal and colleague safety in relation to physical aspects of the job; this would include lone working and carrying of equipment. Ensure all team members' work within the policy.
+ Have regard to the workload of, and the pressure of, professional colleagues and junior team members, taking appropriate action as necessary.
+ Assess report and manage any risk, clinical and non-clinical for the benefit of self, colleagues and clients, in line with Risk Management Policy, particularly in relation to Lone Worker Policy and Health & Safety at Work.
+ Ensure a team philosophy embraces a holistic approach to children, parents, families, and carers.
+ Support team members to negotiate, agree, implement and evaluate planed care with clients, families, carers and other agencies.
Patient / Customer Care
+ Encourage and support team members to practice safely in Nurse Prescribing, demonstrating up-to-date knowledge of prescribed items including side effects and contra-indications and having due regard for security and administration.
+ Empower and support team members to deliver a high standard of care to ensure individuals and their families reach their full potential in Health Promotion and Health Awareness, providing ongoing support to families with emotional, physical and life limiting conditions.
+ Support team members to provide safe care in clients' home including demanding/complex cases that may require advanced assessment and management skills. In addition team members will work within variable conditions, some undesirable and potentially hazardous, delivering care in health centres, GP surgeries, schools, community venues and Family centres as required.
+ Maintain a high standard of infection control knowledge.
+ Support team members in their continuing responsibility for the assessment of physical, emotional and social health needs and the development, implementation and evaluation of programmes of care for individuals and families. Oversee the level of Early Help assessments taking place within the team and encourage staff to act as Lead Professional where appropriate.
+ Maintain a high standard of both written and computerised documentation within agreed guidelines. Ensure that team members have a clear understanding of the HCT record keeping policy
+ At times act as the patients' advocate raising their concerns about services of HCT or other agencies.
+ Follow HCT and Hertfordshire Safeguarding Procedures, including the provision of assessments, reports and attendance at case conferences as required from the team. Have up to date knowledge and training on core group process and other identified work with children and families on the Child Protection Register and liaise with Safeguarding team as appropriate.
Strategic Management
+ Be prepared to participate in the HCT Emergency Plan in response to a major incident as requested.
Service Development, Quality and Improvement
+ Have a good working knowledge of the Quality Assurance framework ensuring sound leadership with Dip Tests (monthly checks/audits of patients' electronic records), Peer reviews (yearly risk-based assessment of services, ensuring care is being given to a consistence high standard) and Quality Wheels (quarterly self-assessment of the service against key quality measures aligned with the five Clinical and Quality Strategy priorities).
+ Incorporate the Trusts Making Things Better Approach to quality improvement through insight, involvement and improvements when leading on quality improvements and change ideas for staff, team service and service users.
+ Lead in the development of innovative programmes to promote good health and participate in Public Health initiatives within the community, working collaboratively with other agencies, as agreed with commissioners.
+ Support and guide clinicians in the development of their skills and knowledge base to effectively lead their respective teams.
+ Lead projects as agreed with the Operational Manager, in line with HCT and national strategies, and personal skills and interest.
Management and Leadership
+ Attend regularly and participate actively in Restorative Supervision in line with HCT Policy.
+ Manage, monitor and support all grades of staff in the team, managing the team's staff appraisal process.
+ To act as a resource within the HCT concerning current development within the service, taking forward initiatives to share best practice, acting as a clinical role model within the team by demonstrating high levels of clinical practice.
+ Take responsibility for own continuous professional development, maintaining a sound level of professional knowledge and competence in line with your Personal Development Plan agreed at annual appraisal. Ensure attendance at all required (mandatory) training.
+ Provide effective mentorship by facilitating a learning environment to encourage the professional development of pre-registration and post-registration students on an ongoing basis. Ensure all team members are up to date with current mentorship training.
Communication and Relationship Building
+ Maintain confidentiality and manage information sensitively, acting as role model for colleagues. Demonstrate discretion and respect when communicating with clients, including those with special needs, colleagues and other agencies.
+ Form and maintain strong communication links with GP practices within area of responsibility, working in partnership with GP colleagues in Practice Based Commissioning.
+ Work in partnership with other agencies in order to meet local and national objectives as agreed with commissioners.
+ Work in partnership with the HCT's Health Promotion department to ensure effective use of health promotion opportunities and resources.
+ Demonstrate advanced communication skills with all team members and stakeholders.
+ Investigate and respond to formal and informal complaints according to HCT policy, working in conjunction with Operational Manager, and participate in peer support and shared learning with other team leaders.
Finance and Resource Management
+ Ensure resources are used as efficiently and effectively as possible, taking into consideration allocation of staff, time management, clinical skills and equipment.
Information Management
+ Positively participate in and promote Clinical Governance, thus ensuring the highest quality of practice is maintained, within the service. Demonstrate the use of evidence based practice and participation in clinical audit.
+ To actively participate in clinical audit, taking the lead as appropriate.
General
+ Maintain up to date registration and revalidation with the Nursing and Midwifery Council, in line with HCT Policy. Provide evidence on an annual basis to this effect.
+ The post holder is involved for periods of prolonged concentration in the development of documentation relating to different incidents, complaints, individual staff members or work practices or procedures in order to achieve a suitable solution, future pathway or record as required.
+ Will be required to communicate difficult and sometimes distressing information effectively, to clients, their families and team members.
+ Be a qualified Specialist Community Public Health Nurse with sufficient knowledge, experience and leadership skills/qualities.
+ Be able to demonstrate advanced communication skills, written and oral, with all team members and stakeholders.
+ Be able to demonstrate the flexibility to effectively and efficiently prioritise workload, balance numerous demands at once and continue to lead, manage and contain the workforce.
+ Demonstrate compassionate leadership as there is clear evidence that this results in more engaged and motivated staff with high levels of wellbeing, which in turn results in high-quality care.
+ Act, at times, as the patient or staff members advocate raising their concerns about services of HCT or other agencies.
+ Hold a full, valid, UK driving licence and have access to a car to use with business insurance (unless the member of staff has a disability as defined by the Equality Act 2010).
