Full Job Description
An opportunity has arisen to join a small team of staff dedicated to improving health outcomes for people with severe mental illness diagnoses. These people will be prescribed antipsychotics and or mood stabilisers. By being involved in the team you will help to monitor and support such people using physical health observations, support with lifestyle choices and mental health reviews. This Service is integral to the overall support provided by our Community Mental Health Services In the role you willsupport the planning and coordination of the physical healthcare needs of patients within the Community Mental Health Pathways; including the requirement for physical examinations and investigations, and the communication of the subsequent plan and intervention to the patient, relatives, carers, MDT, GP, and other organisations where appropriate. You will support system improvement and design, to meet the physical healthcare needs of patients within the Community Mental Health Pathways. You will be expected to be available for consultation, support and clinical supervision to other health professionals working within the pathway and other areas of the Trust where identified as appropriate. You will be responsible for completing assessments, medication reviews and routine reviews. The delivery of a range of high level specialist assessments, identifying risks and management plans in the process.,
- Support the planning and coordination of the physical healthcare needs of patients within the Community Mental Health Pathways; including the requirement for physical examinations and investigations.
- Contribution to the delivery of specialist assessment and evidence based mental health interventions associated with the pathway,
- To complete and or order necessary investigations, and provide treatment and care individually, as part of a team, through supervision of staff, and through liaison with / referral to other agencies, e.g. ECGs, Bloods.
- To undertake health promotion and prevention activities including, for example, educating patients in the side effects of medication and lifestyle choices.
- To draw on a diverse range of knowledge in making decisions to determine evidence-based therapeutic interventions (which will usually include prescribing medication and actively monitoring the effectiveness of therapeutic psychological / lifestyle interventions).
- To make and receive referrals as an autonomous practitioners.
- To utilise expert nursing skills to provide assessment, intervention and clinical guidance as appropriate
- To contribute to the clinical service development.
- To be responsible for an agreed caseload determined as an MDT and reviewed in supervision.
