An opportunity has arisen for a part time physical health support worker to join a vibrant Early Intervention Service on a permanent contract..

The team covers the South and Central area of Manchester and operates from Chorlton House.

The post holder will work alongside the Early Intervention Team covering the South and Central area of Manchester under the supervision of the physical health lead and team manager, undertaking physical health checks, physical health clinics and general health and wellbeing assessments. The post holder will assess physiological observations and promote healthy lifestyles for our service users. The post holder will assist in enabling service users to access the support they need to recover from their mental health and physical health problems, promoting recovery in order to return to everyday living. They will also assist care co-coordinators in working across boundaries of care to plan, implement and evaluate care within care pathways as part of the stepped care model., The post holder will work alongside the Early Intervention Service in South and Central Manchester under the supervision of the team manager, undertaking physical health checks, physical health clinics and general health and wellbeing assessments.
