The service is based in Truro but the successful applicants would be expected to travel around the county to deliver clinics, as well as attending some client's homes in the community. If you choose to take this opportunity to work with us, you will be joining during a period of unprecedented transformation where our services and the Mental Health system as a whole is being redesigned via a process of coproduction with those who currently utilise our services and so know exactly what needs attention and how we might support people in the most meaningful, respectful and effective way.,

  • To ensure provision of holistic nursing assessment and high quality, safe, evidence based care to patients.
  • Assessing, diagnosing, care planning, reviewing all aspects of physical care and supporting mental health nurses and support workers in the community to also do this, upskilling staff when needed.
  • To support the team manager in making strategic improvements to the health and safety of patients through CQUINs and institute of health improvement projects.
  • The post holders will also be encouraged and supported to make improvements to care that they identify on a day to day basis.
  • Help carry out audits to ensure that practise is safe and analyse data and formulate strategies to improve practice within the community teams with the staff.

We're an NHS community and mental health provider Trust based in Cornwall and the Isles of Scilly. We deliver community and hospital-based care to improve people's physical and mental health. We also provide specialist support to people with dementia or a learning disability.
  • We are a people organisation and people matter to us. As part of the team, you'll help support the health and wellbeing of the people who live and visit this beautiful part of the UK. Over 4,000 people make up the Trust. This includes doctors, nurses, therapists, plus admin and support staff. We work in people's homes, in community clinics and bases. Some staff work from one of our 13 community hospitals. Our aspiration is to have great people, provide great care, be a great place to work and a great partner. Approximately 568,000 people live here. A third of people who live in Cornwall are supported by acute hospital services in Devon. As a result, we also work closely with our partners in Devon. In the summer, and during other holidays lots of people choose to visit the area. This increases the numbers of people who use our services.
