Full Job Description

Reporting to the Director of Specialist Services, the Physiotherapist will be responsible for assessment and treatment of adults intheir own homes during their Reablement journey, the expectation will be to work within Norfolk and Waverly ICB. To providephysiotherapy advice, guidance and information to service users, support workers and health and social service professionals.

Roles & Responsibilities

To assess, plan, implement and monitor physiotherapy treatment programmes for Reablement adults within their own homes across all specified areas of Norfolk

To analyse, evaluate and modify treatment in order to measure progress and assure effectiveness of the intervention.

To be professionally and legally accountable for all aspects of your own work to ensure a high standard of clinical care.

To refer and escalate to other staff and agencies within Cera and key external stakeholders as appropriate.

To assist in relevant aspects of health promotion for service users within the Reablement pathway.

To refer to and recommend the provision of aids to assist the service user to achieve their reablement goals.

To establish effective communication networks with service users, support workers and other team members

To be responsible for the effective and efficient organisation of your own time management, prioritisation, workload and the safe use of equipment., To take reasonable care for ensuring your own personal health and safety and that of service users and other staff that may be affected by your acts or omissions.

Ability to be flexible to the needs of the service., 2 years experience working as an autonomous Physiotherapist

Experience in working in the Community

Experience of team working and liaison with other staff groups

Evidence of CPD portfolio

Awareness and understanding of NICE guidelines
