Full Job Description

An excellent opportunity has arisen for a Band 6 Physiotherapist to join the Northwest Surrey Community Rehabilitation Team. In this patient facing post you will work as a member of the multi-disciplinary team, undertaking advanced assessments, clinical diagnosis, and treatment of patients within the speciality who are requiring rehabilitation. These patients often have complex and / or chronic presentations. You will be treating patients across the community in their home environment, working as an autonomous practitioner to undertake a significant specialist clinical caseload, working without direct supervision. You will be well supported within this friendly team, having access to a senior Physiotherapist if required, while also having the opportunity to work independently as clinical work is not routinely evaluated. Within this post, you will be able to further your practice as a Physiotherapist as you will be expected to demonstrate leadership skills and contribute to service
development, monitoring of standards of practice and developing multidisciplinary management of patients.

Some of the principal duties of this role include the following:

· To communicate effectively with all disciplines involved in a patient's care both those from within the organisation and those outside, to ensure effective multidisciplinary working. This will include discussion of patient care, patient progress and involvement in discharge planning.

· To be able to communicate complex and sensitive information to patients, carers and other staff including imparting unwelcome news relating to their rehabilitation potential and gaining consent for treatment.

· To communicate any changes in care plan or treatment approach to physiotherapists and the wider MDT as necessary.

· To ensure timely and effective communication with the Community rehabilitation team manager and senior staff on all professional matters.

· To maintain accurate records in line with the Chartered Society of Physiotherapy standards and organisation and service standards and policies.

· HCPC registration

· B.S.C Hons or diploma equivalent in Physiotherapy

· Full Driver's License - with access to a car for visits
