  • Are you a qualified Physiotherapist who wants to make a difference? Do you want to be involved in the continued development of an innovative integrated service? As an autonomous member of a well-supported therapy team within Nottingham CityCare Partnerships Integrated Care Homes Service, your aim will be to optimise clients independence and prevent hospital admissions. As an NHS Health Professional, you will provide integrated holistic care to a wide range of clients, by in reaching into Nottingham City Care homes to deliver NHS services. The services overarching aim is to ensure that care home residents and staff have an improved experience through high quality care and access to familiar health professionals with the skills to meet their needs. Your caseload will comprise of adults living in care homes who have either complex physical health needs or are living with dementia. In addition to your proven physical therapy skills, you will have the opportunity to develop your knowledge and skills working with professionals from the specialist services, who in reach into care homes giving you invaluable CPD to support your continued learning and development.Main duties of the job This Physiotherapy role is part of the therapy element of the Integrated Care Home Service.This element comprises of both physical health and dementia specialist therapists and Assistant Practitioners, working as an integrated therapy team, supported by a therapy lead. All referrals to the therapy element of the service are triaged by one of the therapy team and assigned to the most appropriate therapist for the patients needs. The Physiotherapist is required to possess a high level of clinical knowledge and up to date experience in this area of work. They are responsible for the holistic assessment of patients and the prescribing of a programme of therapy for individual patient requirements. They will hold a clinical caseload. There are wider therapy meetings, led by the clinical lead, with therapists from other CityCare services who in reach into the care homes. These meetings enable a sharing of knowledge and the development of new and innovative ways of working. The Physiotherapist will be involved in contributing to the evaluation and audits of service provision. They will be required to attend meetings and partake in working groups (as appropriate) in which knowledge regarding therapy interventions to residents in care homes is required, and to represent the wider service as required.
