Physiotherapist - IPU and Outpatients

Leadership Ensure that patient documentation is completed accurately using SystmOne and paper records in accordance with professional standards, hospice policy and legal requirements. Be responsible for managing their own caseload across the inpatient unit, community and outpatient groups and courses. Be responsible for the line management of the Rehab assistant in post. Support the Senior Physiotherapist in the delivery of formal and informal education. Maintain and develop current knowledge of evidence based practice in palliative care and National Standards Framework & guidelines and incorporate these appropriately into clinical practice. Attend relevant meetings as appropriate. Key tasks and duties Along with other members of the allied health professional team and wider MDT work to develop an appropriate rehabilitation programme for clients. Identify and work with patients as early as possible to develop their independence and autonomy with activities of daily living (ADLs) to
maximise their function and engagement in daily life. Constantly assess the on-going physiotherapy needs of the patient, adapting therapy input as their condition changes
