Be responsible for delivery of care in a reablement setting supporting an environment where all team members are responsible for a reablement ethos using a positive risk-taking approach for patients, their carers and families.
To support patients being as independent as possible within their functional limits as part of a multidisciplinary team within a quality framework to ensure that the highest standards of care are met.
To support and deliver innovation within discharge practice and pathways working with system partners including the voluntary sector to ensure that patients experience a seamless transfer of care both into and onward from the unit.
Responsible for the delivery and co-ordination of care to a named group of patients, working within the team to achieve unit and trust goals and promote recognised policies, protocols, and guidelines.
Work as an active member of the Physiotherapy team, along with the other members of the Trusts Multidisciplinary team. This may include liaising with other Trusts, statutory and voluntary organisations e.g. GP's, members of the Biomechanics team, voluntary organisations, carers etc.

+ Develop, monitor and review individualised physiotherapy treatment plans of patients, in collaboration with the integrated team, other agencies and the patient, utilising a range of treatment techniques & equipment to maximise their functional ability.
+ Assess patients' holistic needs, communicating complex and sensitive information to patients and carers as to their assessment, diagnosis, prognosis and treatment plan.
+ Where there are barriers to understanding, such as hearing impairment, mental capacity impairment and other difficulties in comprehension, explanations require adjustments in order to gain understanding, consent and concordance.
+ Be required to use tact and persuasive skills in order to gain the patient's co-operation in their treatment plan.
+ Be aware of assistive technology and refer and utilise as required.
+ Liaise with GPs, Social Services, inpatient teams, other allied health care professionals and the voluntary sector to ensure identified needs are met and care co-ordinated appropriately.
+ To participate in multi-disciplinary/multi-agency meetings as appropriate, e.g. Gold Standard Framework.
+ Ensure own caseload is planned and prioritised according to service targets and patient need.
+ Ensure patient held records are completed for each session/visit/clinic appointment, and that documentation is completed within 24 hours.
+ Demonstrate dexterity and co-ordination when using specialist equipment and fine tools, advanced sensory skills, manual handling and mobilising skills.
+ Contribute to the requisitioning of supplies and equipment through electronic ordering systems.
+ Ensure informed consent is obtained prior to initiating interventions.
+ Will be exposed to bodily fluids, infected material, blood products on a daily basis, therefore must utilise universal precautions and adhere to infection control policies.
+ Be responsible for safe use and maintenance of equipment and supplies
+ To accept clinical responsibility for a caseloads of patients as part of the musculoskeletal physiotherapy team
Professional
The post holder will:
+ Actively contribute to multidisciplinary team meetings and other meetings where appropriate.
+ Support all team members within the local and wider integrated team, respecting them, their roles and contributions.
+ Attend in-house training as required to develop current job role.
+ Maintain own continuous professional development.
+ Remain accountable for own professional actions as determined by the professional bodies.
+ Ensure own caseload is planned and prioritised according to service targets and patient need.
+ Be accountable for work delegated to others within the team, ensuring staff are competent to undertake delegated work.
+ Provide advice / training to less experienced staff and supervise / support band 3 and 4 staff and students where appropriate; contributing to the induction process for new staff/students and contributing to team in-service training programs.
+ Comment on draft policies.
+ Demonstrate leadership skills within own competence.
Organisational
The post holder will:
+ Undertake mandatory training as required.
+ Participate in a Personal Development Review process.
+ Undertake Personal Development Plans for staff members.
+ Be aware of Trust's behaviour framework and ensure behaviours are embedded in role.
+ Ensure Trust's behaviour framework is utilised when managing and supervising staff and that behaviours are embraced and embedded within the team.
+ Take part in clinical supervision as per Trust Policy.
+ Follow Trust policies, and local procedures, and use discretion in interpreting them according to the setting and circumstances.
+ May be required to demonstrate own duties to students, new starters and/or less experienced staff.
+ Provide mentorship for students undertaking pre/post registration course.
+ Ensure absence reporting is undertaken as determined by Team Leader and Trust Policy.
+ Contribute to clinical audit as required.
+ Complete the staff surveys as required.
+ Complete risk assessments and incident forms as required.

NCH&C is hiring several multidisciplinary roles within the state-of-the-art new Willow Therapy Unit. Opening at Norwich Community Hospital in 2024, the new facility will provide a supportive and comfortable environment for 48 patients. The Willow site is currently under construction, the team are currently based at Birch Unit on the Colman Hospital site until Willow opens.
The restorative environment at Willow Therapy Unit will enable patients to make steady, step-by-step improvements and gain independence as they prepare to return to the community. Patients will be supported to play an active role in their own care and feel confident about performing daily activities when they leave.
