Full Job Description

This is an exciting opportunity for a committed, organised and enthusiastic person to join the Physiotherapy team in the north community stroke service. The Fixed term post available is in the north of the county based at Wayside House and will involve travelling across north Warwickshire and Rugby. The post holder will form part of the Physiotherapy team as a technical instructor and will be responsible for providing interventions as a result of a stroke. The post holder will be given the opportunity to work across the community and appropriate training will be offered.

In this post, you will be working alongside other Allied Health Professional's and rehabilitation assistants. Your role will be to help provide timely and targeted assessment and treatment to patients in their own home in line with national stroke guidelines.

We are looking for someone who shares our enthusiasm for patient centred care and is a real team player and more importantly shares a passion for working with people. Excellent communication skills, flexibility, good time management and organisational skills and being a car driver are essential for this post.

You will receive ongoing support through appraisal, supervision and regular team meetings plus access to in-house training., The role of the Assistant Practitioner is to deliver a high quality, caring and compassionate patient care.

The post holder will be undertake assessment, implementation and evaluation of patient care needs for a defined group of patients under the indirect supervision of a registered professional in their specialist area.

The post holders are required to work as part of a team committed to delivering and improving a high standard of nursing care within South Warwickshire NHS Foundation Trust.
