An exciting opportunity has arisen for a Population Health & Care Analyst to join the Population Health and Care team at Lewisham and Greenwich ICS places, where they will play an integral role as part of the Analytics workstream. Lewisham and Greenwich are the only boroughs in South East London to have a population health management information system (Oracle Health Data, formerly HealtheIntent) that is capable of driving change, innovation and clinical effectiveness across the patch. As a programme, we have been able to support and collaborate on incredibly exciting and rewarding projects, some of which include: o Chronic disease management, particularly hypertension (including risk modelling), diabetes (undiagnosed diabetics, non-diabetic hyperglycaemia, and gestational diabetes), CKD, Atrial Fibrillation and COPD o Population Profiles in Lewisham and Greenwich o Projects aimed at tackling health disparities, including establishing the Core20PLUS5, prioritising health checks for
vulnerable patients and a new weight management service tailored for our Black African and Caribbean communities o COVID-19 Vaccination Delivery Group, in partnership with the London Borough of Lewisham o Prevalence of the Vital 5 in Lewisham and Greenwich, in collaboration with King's Health Partners (KHP)o Quality improvement projects, including the use of disease registers to identify those at risk or those living with a health condition., This is an exciting time to join the program as we continue to grow and expand into new fields such as data science and predictive modelling to identify those most at risk of needing care in the future., the health and care needs of the population and develop new integrated ways of working to support those needs.

The successful candidate will support clinicians and commissioners as they develop their areas for investigation, work with them to define their key questions, translate these into data queries, and produce intelligent analytics to help inform key decision-making and improve health outcomes for the population. We are looking for an experienced data analyst to join and support the team, working across Lewisham and Greenwich Health and Care Partnerships. The person we are looking for will need to have a wide range of knowledge and skills including: o Strong experience of working within health or social care data across organisations, synthesising information from a range of sourceso Experience of working with data in acute, primary care, community, public health data, mental health and social care data settingso Excellent programming and analytical skills using industry standard applications (MS SQL, Tableau, Business Objects) o Excellent communication skills with experience of
communicating complex information to a lay audience o Understanding of information governance legislation and data ethics o Ability to work autonomously, prioritising your workloads

o Experience working in a primary care setting in EMIS will be an advantage

Lewisham Health and Care Partners (LHCP) work across partner organisations as the main health and care commissioners and providers in Lewisham. Healthier Greenwich Partnership (HGP) work across partners to bring together health and care providers in Greenwich. LHCP and HGP are supporting delivery of a population health analytics solution, using South East London ICB as their anchor institution. This post will support both Lewisham and Greenwich analytics.LHCP and HGP will provide a system-wide leadership and strategic direction for the transformation of health and care across Lewisham and Greenwich. A core element of this work is the development of the Oracle Health Data platform, which has been pivotal in driving change, innovation, and clinical effectiveness across the Lewisham borough.
