Full Job Description

We are seeking a dedicated and experienced Transition Practitioner to join our team. This role is part of a project aimed at supporting children with SEND who are transitioning into Year R without an Education, Health and Care Plan (EHCP). The goal is to provide early interventions and services to prevent the escalation of needs and ensure a smooth transition into mainstream school.   • In-depth knowledge of child development: Understanding the early years of child development is crucial, especially for children with SEND.
  • Experience with families and pre-school children: Practical experience in this area will help you connect and support families effectively.
