Full Job Description

Tower Hamlets Diagnostic Memory Clinic are looking for an enthusiastic and competent mental health nurse, with a specialist interest Dementia Diagnostics and Post Diagnostic care, to deliver our post diagnostic support offer.

This is a Secondment covering maternity leave.

The Tower Hamlets Memory Clinic provides a dynamic, innovative and responsive service for the population where dementia diagnosis rates are increasing.

The clinic provides high quality comprehensive assessments and diagnosis for people experiencing cognitive decline by working actively in partnership with service users, carers and relatives. This includes high quality post-diagnostic support and intervention that promotes independence, social inclusion and maintains quality of life.

You will be working collaboratively with the wider team to build and maintain our post diagnostic support offer. You will be responsible for supporting a caseload of service users, who have recently been diagnosed, to develop person centred, independence maintaining care plans. This will also include a degree of shared oversight and facilitation of our post diagnostic treatment groups and activities.

You will join a motivated, friendly and supportive multi-disciplinary team who continue to make a positive difference for our local community.
