Practice Nurse (12 month Fixed Term Contract)

Job summary Working as a multi disciplinary team member, providing high quality nursing care to patients, carers and families registered at LSMP. Make relevant and appropriate assessments of our patients physical and psychological needs. Demonstrate safe clinical decision-making skills to meet these needs by working independently or to practice policies and protocols. Patients should feel that they have been dealt with in a friendly, courteous manner and that all that can be done for them has been done. The nursing team works in partnership with the medical team or multi-disciplinary team to provide care to patients with undifferentiated conditions (minor illness and minor injury) at the point of contact, or referring/signposting them to alternative appropriate services. The team also offers a range of clinical services which are; Contraception and sexual health advice and screening Contraceptive intrauterine device and subdermal implant fitting Cervical cytology Asthma management
Diabetes management Wound management Travel advice and vaccinations Weight management advice Ear care Audiometric testing Health screening Health checks Blood pressure checks Phlebotomy clinics Support to GPs undertaking private medicals Management of the childhood immunisation programme Minor Illnesses Skin and Acne Triaging and eConsults Vaccinations
