Practice Nurse

We are looking for an experienced Practice Nurse, with experience in health prevention and long term condition. Diplomas in Asthma, Diabetes, COPD & Hypertension advantageous.

Working in accordance with the UKCC Code of Conduct, the post-holder will provide assessment, treatment, screening, health education services and advice to patients.

The post-holder will work with reference to Practice agreed protocols and guidelines, providing nursing treatments to patients both independently and in participation with nurse colleagues and general practitioners.

The post-holder will have the required level of training and competence and will work within patient group directions where these are appropriate.

Main duties of the job

Assessing routine wounds including trauma

To support good wound management in complex situations (e.g. use of Doppler technique)

Patient Health Checks / Health Screening

Recognise issues and gather sufficient information to refer where appropriate. This will

include womens and mens health, sexual health and vulnerable adults.

Undertake routine screening (i.e.) smears

Provide any monitoring felt to be required after assessment.

Obtain appropriate information using lifestyle questioning approach and identify health promotion issues.

Work with patients to develop appropriate management plan for health problems or issues that are identified.

Risk Assessment

Recognising issues and gathering sufficient information to refer (e.g. drugs;

domestic violence; child protection; vulnerable adults and social problems)

Travel Health

Providing travel risk management by administering injections in accordance with guidelines

Identifying problems

Provision of specialist and evidence based advice to patients

Immunisation (Adult and Child)

Administering appropriate immunisation autonomously with the PGD instructions

Working with patients with more complex immunisation issues.

Minor operations

Assisting the general practitioner and/or undertaking some surgical procedures
