Practice Nurse

We have an exciting opportunity for an experienced Nurse to join our team at Mulberry/City practice, on a permanent 17 hours per week contract., We are looking for a reliable, enthusiastic and motivated Practice Nurse to work with us. A nurse who can provide excellent care and has a true dedication for improving health outcomes for patients.

The successful candidates will be a registered nurse, ideally with experience of working in General Practice.

The post holder willwork as a member of the Practice Nursing Team and be responsible for acting within their professional boundaries to provide care for the presenting patient including screening, clinical assessment, diagnosis, treatment and follow up of care; demonstrating safe clinical decision making and expert patient care.

They will be involved in all areas of Chronic Disease Management (Asthma, Diabetes, COPD etc.)

The work is interesting, often challenging but hugely rewarding. The team is very mutually supportive, with excellent experienced clinical, management and administrative support on site.

On appointment you will have a full induction into the service and support will always be available when needed from our Clinical Nurse Lead.

As well as providing a unified voice for general practice in the city, we play an active role both within our Accountable Care Partnership (ACP) and the wider South Yorkshire and Bassetlaw Integrated Care System (ICS).
