Practice Nurse

This is a role for a Registered Physical Health Nurse or General Nurse or Practice Nurse to work at Three Valleys Hospital, which is part of Elysium Healthcare. You will be responsible for promoting health and wellbeing for people with complex mental health needs, working in a multidisciplinary team. The role involves leading and coordinating the physical health needs of service users, monitoring observations and assessments, and developing and supporting physical health interventions., As a qualified Practice Nurse, you will lead and coordinate all aspects of the physical health needs of service users, while also supporting your colleagues and promoting good teamwork with the multidisciplinary team, such as GPs and other physical health departments. You will be responsible for monitoring observations and assessments to identify any actions that need to be addressed and documented. You will adhere to the NMC Code of Conduct, develop and support physical health interventions, engage in practice development and audit activities, and work collaboratively with primary health and other physical health departments.
