Chronic Disease Management Manage chronic disease clinics for conditions such as diabetes, asthma, COPD, and hypertension. Conduct regular reviews and assessments of patients with long-term conditions, adjusting treatment plans and providing education to optimize self-management. Immunisations and Vaccinations Administer vaccinations and immunizations according to national guidelines and practice protocols. Health Education and Promotion Provide health education and promotion to patients on topics such as smoking cessation, weight management, nutrition, and exercise. Support patients in making lifestyle modifications to improve their health outcomes and prevent disease progression.
Patient Care Coordination Coordinate care for patients with complex health needs, liaising with other healthcare professionals and community services as necessary. Collaborate with multidisciplinary teams to develop and implement care plans that address the holistic needs of patients. Documentation and Record Keeping Maintain accurate and up-to-date electronic medical records, documenting all patient interactions, assessments, and interventions. Ensure compliance with data protection regulations and confidentiality policies in the handling of patient information. Professional Development and Quality Improvement: Participate in ongoing professional development activities to enhance clinical knowledge and skills. Contribute to quality improvement initiatives within the practice, identifying areas for improvement and implementing best practices to enhance patient care. Cervical Screening (Pap Smears) Perform cervical screening procedures, Obtaining informed consent, Explaining the procedure to
patients, Collecting cervical samples, Ensuring sample quality, and Providing emotional support to patients throughout the process.
