Full Job Description
Practice Nurse required at Northlands Surgery, experience preferred. 3 days per week: Monday, Wednesday, Friday. We are looking to recruit a part time practice nurse to join our supportive and friendly team on a part-time basis. We can offer between 18 and 28 hours per week over 3 days. We can be flexible with start and finish times, depending on what you are looking for. We are looking for experience in chronic disease management, immunisations; childhood vaccinations and immunisations; travel advice per national recommendations, and other Practice Nurse duties.Main duties of the job Chronic disease management including the diagnosis, monitoring and development of patients individual management plans. Health promotion and wellbeing. Implement and evaluate individual treatment plans for chronic disease patients. Identify, manage and support patients at risk of developing long-term conditions, preventing adverse effects on patients' health. Provide routine nursing care to patients as required in accordance with clinical-based evidence, NICE and the NSF. Provide wound care (ulcer/Doppler etc.) to patients. Provide immunisations (childhood, seasonal etc) to patients. Provide travel medicine services. Provide Cervical Cytology screening. Request and process pathology results as required. Maintain accurate clinical records in conjunction with existing legislation. Ensure read codes are used effectively and support with achievement of QOF targets. Chaperone patients where necessary. Support the team in dealing with clinical emergencies and safeguarding matters. Support patients in the use of their prescribed medicines or over-the-counter medicines (within own scope of practice). Liaise with external services/agencies to ensure the patient is supported appropriately (vulnerable patients etc.).
